Universal
Fuel Card
A mandatory
statewide fuel management program was implemented in 1998,
and a part of this program is the new universal fuel credit
card. The new card offers benefits such as electronic cost
savings resulting from automatic federal and state tax exemption.
The new
fuel card works in the following manner. A new universal fuel
card is assigned to a vehicle. Each vehicle will have one
card associated with it, and one account number will be assigned
to each card. The department will be responsible for assigning
a Driver Identification Number, which they may need to enter
at the fuel pumps depending on the level of technology available
at the fueling site. Each card will have an unlimited dollar
amount for fuel.
New cards
can be requested by completing the appropriate form included
below and mailing it or faxing it to the Procurement Office
at 706-542-7035. This same form may also be used to cancel
existing cards or to request replacement of cards due to the
card being damaged, stolen or lost. The other form should
be used when requesting driver identification numbers for
individuals who will be driving the vehicles. Complete the
top of the driver listing form and list the individual's names
and addresses. Procurement will assign the Driver Identification
Number (PIN) and provide the point of contact at the department
with the numbers for dissemination to the individuals.
If you
have questions relating to these forms or to the policies
and procedures governing the fuel card, please contact
Lynn Stephens at 706-542-7108 or
