Statement
A
"Campus Mail Statement"
form must accompany each piece, bundle, or bag of mail sent
to Campus Mail for postage and delivery to the U.S. Post Office.
Each form is bar-coded with the department's 12 digit account
number and this account will be charged daily. To efficiently
process outgoing mail, the department should:
Ensure
that the name of the department/unit to be charged and the
appropriate account number to be charged is shown on the mail
statement with the barcode. Indicate the number of pieces
of mail for the appropriate category included in the mailing.
A separate form for each category of mail is required as well
as for letter size and large envelopes (flats) in each category.
The individual authorized by the department/unit to process
mail must sign and date the form. The signature must be legible
and a telephone number is requested in the event that person
needs to be contacted by Campus Mail. Mail received at Campus
Mail without a "Campus Mail Statement"
form or a form without the account number or signature, may
be delayed until appropriate information is received.
